R/2018/151, launched 31 July 2018, 12,182 vehicles affected across the models it covers. Also covers the Corolla
Defect: Affected vehicles received a recall remedy in which the replacement inflator may have been installed in an incorrect orientation. This may prevent the front passenger air bag from inflating as intended during deployment as designed.
Remedy: Affected vehicles will have the inflator checked and corrected if it is necessary.
FUEL MAY LEAK / RISK OF FIRE
R/2016/152, launched 22 September 2016, 72,891 vehicles affected across the models it covers. Also covers the Lexus CT2000H, Prius
Defect: The vehicles are equipped with a fuel evaporative emission control unit (canister) assembled to the fuel tank. There is a possibility that a crack could develop on the resin channel for the emission gas flow due to an improper shape of the channel edge at that location. The crack could expand over time and eventually fuel may leak from the crack when the vehicle has a full tank. Fuel or fuel vapour in the presence of an ignition source could increase the risk of a vehicle fire
Remedy: Recall the vehicles that are likely to be affected and replace the fuel suction plate sub-assembly.
HEADLIGHT MAY MALFUNCTION
R/2016/169, launched 25 August 2016, 14 vehicles affected across the models it covers. Also covers the ADE186, NDE180, NRE180, ZRE185, ZWE186
Defect: Headlamps manufactured to the incorrect specification may have been fitted and not may distribute light correctly.
Remedy: On affected vehicles replace headlamp units.
RISK OF FIRE
R/2015/206, launched 1 December 2015, 123,306 vehicles affected across the models it covers. Also covers the Camry, Corolla, Highlander, RAV4, Urban Cruiser, Yaris
Defect: Affected vehicles are equipped with a driver’s side Power Window Master Switch (PWMS) containing sliding electrical contact modules. These modules may not have been incorrectly coated during the manufacturing process. This may cause a short circuit which could cause the switch to overheat and melt
Remedy: Inspect switch and apply coating. If switch indicates signs of malfunction it should be replaced.
LOSS OF POWER
R/2015/152, launched 10 August 2015, 24,734 vehicles affected across the models it covers. Also covers the Prius
Defect: The inverter assembly may overheat which will cause warning lamps to illuminate and the vehicle to enter a fail safe mode. Fail safe allows the vehicle to be driven on reduced power to a place of safety. However it is possible for the hybrid system to shut down causing the vehicle to stop which increases the risk of a collision.
Remedy: On affected vehicles reprogramme the Motor Generator and Power Management ECUs.
PASSENGER SIDE AIRBAG MAY FAIL TO DEPLOY CORRECTLY
R/2015/096, launched 15 June 2015, 542,751 vehicles affected across the models it covers. Also covers the Avensis, Corolla, Is, Is-C, Is-F, Lexus SC430, Picnic, Yaris
Defect: The subject vehicles are equipped with front passenger airbag inflators which could have been assembled with improperly manufactured propellant wafers. Improperly manufactured propellant wafers could cause the inflator to rupture and the front passenger airbag to deploy abnormally in a crash. This could result in increased risk of injury to the occupant and the possibility of a fire.
Remedy: Recall the vehicles that are likely to be affected to replace the airbag.
RISK OF FIRE
R/2014/160, launched 7 January 2015, 7,884 vehicles affected across the models it covers. Also covers the Corolla, Urban Cruiser, Yaris, iQ
Defect: Oil may accumulate in the turbo charger and in certain circumstances flow into the combustion chamber. This may cause engine damage. In extreme cases oil can be ejected from the engine and if it comes in contact with an ignition source may result in a fire.
Remedy: Depending vehicle diagnosis modify intercooler layout to improve air flow; upgrade ECU software; upgrade turbo charger and/or vacuum pump
FIRE MAY OCCUR
R/2012/118, launched 26 October 2012, 138,049 vehicles affected across the models it covers. Also covers the RAV4, Yaris
Defect: It is possible moisture or other fluids may enter the drivers window and contaminate the main power window switch. If this occurs and the switch is damaged it could overheat and cause it to melt and/or smoke. If this situation is not identified it could result in a vehicle fire.
Remedy: Recall all affected vehicles and check the switch. If the switch is not damaged a waterproof cover will be fitted. If the switch is damaged it will be replaced and a waterproof cover will be fitted.
REAR SUSPENSION ARM MAY SEPARATE
R/2012/085, launched 12 September 2012, 65,863 vehicles affected across the models it covers. Also covers the Avensis, RAV4
Defect: If the rear suspension arm is adjusted as part of a wheel alignment or other repairs whilst in service it is possible that backlash can occur. This can allow corrosion to develop and cause the threads to deteriorate. Ultimately this could result in failure of the arm with a resultant loss of control of the vehicle.
Remedy: Recall all affected vehicles to check the torque on the arm. Where necessary the arm will be replaced.
ACCELERATOR PEDAL MAY FAIL TO RETURN TO IDLE
R/2010/010, launched 6 February 2010, 174,426 vehicles affected across the models it covers. Also covers the Avensis, Aygo, RAV4, Verso, Yaris, iQ
Defect: It has been identified that on affected vehicles condensation resulting from cold-weather use of the heater or other causes may form on the worn surface of a component within the throttle pedal assembly known as the friction device. If this occurs there is a possibility that in the worst case the accelerator pedal may return slowly or does not return to the engine idle position.
Remedy: Recalled vehicles will have a precision-cut steel reinforcing bar fitted into the accelerator pedal assembly which will create a space in order to reduce the surface tension between the friction device and a component referred to as the pedal arm. The subject bar also strengthens the reaction force of the spring that returns the accelerator pedal to it's non-pressed position.
REAR BRAKE CALIPERS MAY DETACH
R/2008/189, launched 28 January 2009, 8 vehicles affected across the models it covers
Defect: It has been identified that the rear brake caliper retaining bolts may become loose due to their being tightened to an insufficient torque. This would in the first instance; result in an unusual noise and in the worst case detachment of the caliper which may result in the breakage of the brake disc.
Remedy: Recalled vehicles will have the near and offside brake caliper retaining bolts replaced.
