R/2017/087, launched 24 March 2017, 56,143 vehicles affected across the models it covers
Defect: It has been identified that vehicles equipped with a single stage inflator driver’s front air bag then there is a possibility that the density of the propellant in the inflator could be reduced over time from the effects of high temperature cycling and moisture within the propellant and its sealed container. This may cause the propellant to degrade which if this occurs then the inflator could rupture in the event of the deployment of the driver’s front air bag.
Remedy: Recall all affected vehicle to replace the driver's front air bag.
DRIVER'S SIDE AIRBAG INFLATOR MAY RUPTURE
R/2016/236, launched 9 January 2017, 1,248 vehicles affected across the models it covers

Defect: The subject vehicles are equipped with front passenger airbag inflators which could have been assembled with improperly manufactured propellant wafers. Improperly manufactured propellant wafers could cause the inflator to rupture and the front passenger airbag to deploy abnormally in a crash. This could result in increased risk of injury to the occupant and the possibility of a fire.
Remedy: Recall the vehicles that are likely to be affected to replace the airbag.
RISK OF FIRE
R/2014/160, launched 7 January 2015, 7,884 vehicles affected across the models it covers. Also covers the Auris, Urban Cruiser, Yaris, iQ
Defect: Oil may accumulate in the turbo charger and in certain circumstances flow into the combustion chamber. This may cause engine damage. In extreme cases oil can be ejected from the engine and if it comes in contact with an ignition source may result in a fire.
Remedy: Depending vehicle diagnosis modify intercooler layout to improve air flow; upgrade ECU software; upgrade turbo charger and/or vacuum pump
PASSENGER SIDE AIRBAG MAY FAIL TO DEPLOY CORRECTLY
R/2013/048, launched 17 April 2013, 85,405 vehicles affected across the models it covers. Also covers the Avensis, Lexus SC430, Yaris
LOSS OF STEERING
R/2012/145, launched 3 December 2012, 61,910 vehicles affected across the models it covers. Also covers the Avensis, Prius
Defect: It is possible that the splines of the extension shaft on vehicles equipped with Electric Motor assisted Power Steering (EMPS) may not have received the correct hardness treatment. This can cause the spline to distort and wear and eventually result in loss of steering.
Remedy: Recall affected vehicles check and if necessary replace the shaft.
STEERING MAY FAIL
R/2006/103, launched 13 September 2006, 39,372 vehicles affected across the models it covers. Also covers the Avensis, Prius

R/2004/168, launched 25 April 2005, 6,936 vehicles affected across the models it covers. Also covers the Celica, MR2
Defect: Due to an improper washing process of a metal part within the brake master cylinder there is a possibility that the groove for installing the rubber seal ring which is located at the end of the master cylinder may be corroded. In this condition brake fluid may leak from the seal or a small amount of air may enter the master cylinder which could lead to an increase in vehicle stopping distance.
Remedy: Vehicles that are likely to be affected will be checked and a quality assured part fitted if necessary.
PASSENGER FRONT AIR BAG CONCERN
R/2003/041, launched 18 February 2003, 1 vehicle affected across the models it covers
Defect: The front passenger airbag inflator module check valve pin is missing in the affected vehicle. In the event of airbag activation the partition which separates the inflator and the bag may not be properly broken resulting in insufficient pressure to properly deploy the airbag. This could result in injury to the front seat passenger in the event of a frontal impact.
Remedy: Replace the inflator module.
TWO GEARS MAY BE SELECTED AT THE SAME TIME
R/2002/171, launched 16 December 2002, 50 vehicles affected across the models it covers. Also covers the Verso
Defect: Transmission output shaft critical dimension may be shorter than the specification. In the worst case this could result in two gears being selected at the same time and cause the gearbox to lock up.
Remedy: Check transmission serial number and replace any gearbox found to be within the affected range.
BRAKE PEDAL STROKE MAY INCREASE
R/2002/098, launched 11 July 2002, 4,025 vehicles affected across the models it covers
Defect: Certain brake master cylinder reservoir cap air vents are blocked. If the vehicle is driven in this condition for long term a vacuum is created in the reservoir tank as brake pads wear. There is a possibility that this vacuum may cause the brake pedal stroke to increase.
Remedy: Recall affected vehicles and replace reservoir cap if faulty.
REAR BRAKE AND HUB SECURITY CONCERN
R/2002/052, launched 20 May 2002, 6,617 vehicles affected across the models it covers
Defect: It is possible for the bolts to be loose that join the rear brake and hub assembly to the rear axle carrier due to insufficient tightening. 1 or more of the 4 bolts could come loose causing an abnormal noise from the rear wheel axle area. In the event that all 4 bolts loosen completely separation of the rear brake and hub assembly from the rear axle carrier may occur which in the worst case could lead to a loss of vehicle control.
Remedy: Recall affected vehicles and check the rear brake and hub securing bolts for correct tightness.
